Oral health education and prevention: why information alone does not work

Oral health education and prevention: why information alone does not work

Every practice gives advice. Far fewer change behaviour. The difference is rarely the information — it is how it is delivered.

ICDAS: describing what you see

ICDAS is a scoring system for caries that records the stage of the lesion rather than a simple yes/no. Its practical value is consistency: two clinicians looking at the same tooth should write the same number.

For the assistant, ICDAS matters in three places: photographing and documenting, explaining to the patient what stage they are at, and tracking change over time.

Fluoride: dose, not slogan

Fluoride works. The variables are concentration, frequency and delivery:

  • Toothpaste concentration should match the patient’s caries risk.
  • High-fluoride products are for high-risk patients, not for everyone.
  • Application needs to be timed and recorded, not improvised.

Plaque control: teach one thing

A patient who leaves with four instructions follows none. Choose the single behaviour that matters most for that patient — interdental cleaning, or the timing of brushing, or the frequency of sugary drinks — and make that the whole conversation.

Motivational interviewing in practice

Motivational interviewing is not persuasion. It is a structured conversation that helps the patient say the reason for change in their own words.

Three moves you can use at the chair:

  • Ask permission: “Can I show you something I noticed?”
  • Reflect, don’t correct: “So brushing at night is the one that slips.”
  • Ask for the plan: “What would make that easier this week?”

Notice what is missing: lecturing. The patient leaves with a plan they said out loud, which they are far more likely to follow than one you announced.

Prevention is a workflow, not a lecture

Recall intervals, risk assessment, documented advice and a follow-up question at the next visit. Prevention that is not recorded did not happen.
